New in 2023, the Diabetes Center of Excellence is located at our Lisbon Ave. Health Center location, combining Progressive’s quality primary care with specialized support to create a one-of-a-kind service for those living with diabetes.

We are honored to receive the 2022 Performance Excellence Award from the WPHCA. This recognition reflects our team’s strong commitment to building and implementing a comprehensive care management system that improves how we support patients living with diabetes. By combining data-driven strategies with innovative, patient-centered care, we strive to achieve better health outcomes and create a more seamless healthcare experience for the communities we serve.
In 2021 Progressive eliminated racial and ethnic disparities in diabetes control (as defined by the Uniform Data System). At the end of the year, 23% of the 1,100 African American patients with diabetes were uncontrolled or without a test compared with 22% of white patients.
