We believe that your health is influenced by more than just what happens in one of our health centers. Our dedicated team of Social Workers, Behavioral Health Navigators, Community Health Workers and Financial Counselors works together to ensure you have the support you need to lead a stable, healthy life. By partnering with government agencies and local organizations, we connect you with essential services that you may not have known were available to you.

Healthcare Access
Navigating Medicare, private insurance and applying for health insurance or the Sliding Fee Program.

Daily Essentials
Securing transportation, food access, and energy assistance.

Stability & Safety
Finding resources for housing, employment, and childcare.

Specialized Care
Accessing AODA (Alcohol and Other Drug Abuse) treatment and other specialty health programs.

If you need medical services we don’t provide, our staff works with local experts to help you get the referrals you need. We also take part in the Specialty Access for Uninsured Program (SAUP), so patients without insurance can receive specialty care. If you’re unsure about referrals or need help, talk with your provider to go over your choices.
