In September 2022, Progressive CHC received the 2022 Performance Excellence Award from the Wisconsin Primary Health Care Association (WPHCA) for its work in diabetes at the 2022 Fall WPHCA Learning Session. The Diabetes Clinical Care Management team was nominated for patient engagement, education, and teach-backs to ensure that patients can self-manage their diabetes. This effectively eliminated racial and ethnic disparities in diabetes control in 2021.
Progressive CHC received two nominations for the award including one from the American Cancer Society, a longtime partner, for colorectal screenings. ACS’ nomination included these words: “[They’re] a fantastic team who truly provides patient centric and culturally competent care to advance CRC screening.”
