Patient Information

Progressive Community Health Centers welcomes patients of all ages and incomes, regardless of insurance status or where you live.  We accept most private insurance plans and HMOs, including Medicaid and Medicare. Patients may apply for our Sliding Fee Program if they do not have insurance, or if insurance does not cover all of their health costs. Patients are expected to bring insurance information with them to each appointment.

New Patients

We are currently accepting new patients at all locations. To schedule an appointment, please call (414) 935-8000. Walk-in patients without an appointment may also receive care depending on provider schedules for that particular day, but are encouraged to call ahead of time.  If you are a new patient coming for your first visit, please arrive 15 minutes early to fill out paperwork and complete important forms. Also, bring the following:

  • Insurance information
  • A list of medications you are taking
  • Any medical records from previous/other health care providers

Benefits Enrollment Assistance

Financial counselors are available at all locations to help you apply for Medicaid, private insurance via the Health Insurance Marketplace, or our Sliding Fee Program.  For more information about the Affordable Care Act and your health insurance options, click here or call (414) 935-8000 to make an appointment with a financial counselor.

Sliding Fee Program

Our Sliding Fee Program helps people who have limited or no health insurance. Patients will be charged for services based on their income and the number of people in their household.  To apply for the Sliding Fee Program, you must meet with a financial counselor and provide proof of income and proper identification.

To apply for the sliding fee program, you must provide the following documentation:

  • Proof of income (one of the following)
    • Check stub
    • Letter from employer verifying wage
    • Copy of your most recent federal tax return
  • Identification (one of the following)
    • Driver’s license
    • State identification card

Payment Options

Patients are responsible for all charges not covered by health insurance.  Co-payments and sliding fee payments are expected at the time of service.  Cash, personal checks and credit cards (Visa, MasterCard, Discover) are accepted at all locations.
