Working together to reduce avoidable emergency department visits

The Wisconsin Collaborative for Healthcare Quality is an Aligning Forces for Quality alliance and a partner in the Milwaukee Health Care Partnership’s Emergency Department Care Coordination Initiative—a community effort that uses care coordination and health IT to connect Medicaid and uninsured patients who visit the ED with a primary medical home.
Patients who kept their first medical home appointment used the ED 44 percent less frequently in the six months post-appointment compared to the six months pre-appointment.
