Progressive CHC Featured on Wisconsin Public Radio

Progressive CHC was featured in a recent story that aired on Wisconsin Public Radio which highlights our agency’s participation in the Milwaukee Health Care Partnership’s Emergency Department Care Coordination (EDCC) program.

The goal of the EDCC program is to reduce inappropriate use of emergency room services at local hospitals and connect patients with primary care medical homes across the community.  Patients who show up at the ED for non-emergent issues can be referred to Progressive CHC, in addition to other safety-net clinics across Milwaukee.

The patient featured in this story, Cornelius, was referred to our clinic several years ago via the EDCC program.  Prior to coming to our clinic, he suffered from uncontrolled diabetes.  He never sought help for his chronic conditions until they became emergencies.  Once he was introduced to our clinic, he started to come in regularly for routine tests and follow-up care.

Cornelius is one of hundreds of patients referred to our clinics each year as part of the EDCC program.  We are proud to participate in this important initiative and look forward to continuing to create access to health care for the underserved in our community.
