Progressive Community Health Centers is recruiting to fill an AmeriCorps position in the role Patient Outreach Coordinator in Milwaukee, Wisconsin. This is a limited term employment opportunity with Wisconsin HealthCorps beginning in September 2019 through August 2020. HealthCorps members help improve the overall health of Wisconsin’s communities by increasing knowledge of healthy behaviors, and providing some of the state’s most vulnerable residents with the means to access health care. During their year of service members provide information to individuals on access to care , health insurance options, benefit programs, and health center/ host site services, perform patient call-backs, prepare materials for both individual patients and communities, and teach healthy lifestyle courses to community members- covering a variety of topics such as: healthy eating habits, chronic disease maintenance, and public health concepts.

MEMBER BENEFITS: living allowance, insurance coverage (health, dental and vision), child care assistance, education award, loan forgiveness, and membership in the Wisconsin Public Health Association. Benefits are contingent on a Member’s eligibility and successful completion of 1700 hours of service during the program year.
