Clinic Location Updates

We have some important news to share about our clinic locations that went into effect on August 31, 2020. If you have any questions about your appointment location, call us at (414) 935-8000.
  • The Hillside Family Health Center has officially reopened. If you have an appointment with Tamyra Trice, PA please go to 1452 N. 7th Street.
  • The Kilbourn Avenue Health Center has permanently closed. However, your access to care will continue. If you have a future appointment with Lillian Oduwole, FNP you will now see her at the Lisbon Avenue Health Center (3522 W. Lisbon Ave).
  • The Lisbon Avenue Health Center and 12th Street Urgent Care locations will continue to operate as normal.
